Supporting Successful Transition to Postsecondary Education and Adult Employment

The Office of the Secretary of Higher Education (OSHE) presents the County College-Based Center for Adult Transition Grant, a project administered by and in partnership with OSHE, located at Bergen Community College’s Paramus Campus.

Our Mission

The Center for Adult Transition is dedicated to providing individuals with Intellectual Disability (ID) and Developmental Disability (DD) and their families the resources, support and guidance to encourage the greatest level of independence possible.
The Center collaborates with and supports individuals with ID/DD, and their families to promote access to community-based services and resources in the areas of education, training, and employment.

An Individualized Approach to Transition Planning

The Center for Adult Transition utilizes a Person Centered Planning model to help participants identify the specific services, supports, and resources they need to reach their self-determined postsecondary education and employment goals.
Once the individual identifies their path, they are given options for coursework, vocational training, and resources necessary to achieve successful outcomes.
Through personalized programming and assistance with navigating county and State-based resources, participants of the Center become better prepared to succeed in today’s competitive workforce and live more independent and fulfilled lives.

Resources

As a resource hub, the Center for Adult Transition has established partnerships with providers whose mission supports successful transition outcomes for adults with ID/DD.
By collaborating with community partners, the Center offers guidance and support to individuals and their families with navigating the various local, State-based, and other applicable resources available to persons with disabilities.
Our partners also work with the Center to provide additional inclusive social opportunities, family to family support, and pre-employment resources.

Summer Transition Boot Camp

The Summer Transition Boot Camp is a college-based program for young adults up to age 24 with ID/DD, made available and paid for through BCC’s partnership with The Office of the Secretary of Higher Education, through the County College-Based Center for Adult Transition Grant.
Participants of the summer program attend workshops designed to help them identify their strengths, interests, and postsecondary goals, build communication skills and professional etiquette, and enhance collaborative problem solving through participation in an inclusive STEM Hydroponic Service Learning Project.
Students may receive support connecting to local and State-Based resources through Bergen Community College’s campus partnership with Rutgers Community Living Education Project (CLEP).
The goal of the summer program is for students to leave with a better understanding of their individualized postsecondary interests and options and with an enhanced awareness of the support they may need for more successful transition to postsecondary education and adult employment.

Summer 2024 Transition Boot Camp Dates and Application Process

  • Schedule

    • Date: July 8, 2024 – July 25, 2024.
    • Days & Time: Monday through Thursday, 9:00 A.M – 12:00 P.M.
  • Application Process

    • Applications will be accepted through June 1, 2024, or until the program is fully enrolled;
    • Interview and letters of recommendation required;
    • Space is limited to 15 participants.
