Bergen Community College offers many different resources to support our students and the Faculty and Staff who interact with them on a daily basis.  Having a clear and concise idea of where to report different types of incidents will help the College respond in the most effective way for everyone involved.

Any member of the Bergen community can submit one of the reports below.  Use the examples to best determine the type of incident you would like to report, and then click on the link to access the online incident report.

Incident Report Form

The Code of Conduct governs the behavior expected of our students, and the process that is followed when those expectations are not met.

Violations of the Code of Conduct may include:

  • Severe/repeated classroom disruptions
  • Fights/threats
  • Assault/sexual assault
  • Damage/theft of property
  • Bullying/harassment
  • Drug/alcohol use/possession
  • Cheating/Plagiarism

Behavioral Intervention Team Form (BIT Report)

The Behavioral Review Team (BIT) is a cross-functional group of College officials who review, evaluate and respond (if necessary) to reports of “Students of Concern” focused on the prevention of disruptive behavior and providing early interventions.

The Behavioral Intervention Team is not a crisis emergency response team, rather seeks to implement proactive strategies before an event becomes a crisis. The team will initiate responses and actions for those within the college community who show signs of serious emotional distress, or behaviors that are disruptive/ or harmful to themselves or others.

Concerning behaviors may include:

  • Changes in appearance and/or behavior
  • Perceived mental health issues*
  • Perceived threatening/self-injurious behavior (writing/talking of: harm/cutting/suicidal ideations) *
  • Perceived alcohol or drug concerns*
  • Perceived medical or health issues*
  • Relationship violence and/or issues*
  • Sudden, extended and/or unexplained class absence

Academic Integrity Report

The Code of Conduct governs academic misconduct.

Students are required to exemplify Academic integrity in all of their dealings and interactions. Bergen Community College is committed to academic integrity-the honest, fair, and continuing pursuit of knowledge, free from fraud or deception.

Pipeline/Academic Early Alert Referral Form

Pipeline is an early alert system designed to promote student success and retention by identifying students who are at risk and experiencing academic, personal difficulties.

This system enables Bergen administrators to identify students who are at risk and begin intervention with students as soon as a problem is identified. It also allows us to track all contacts and intervention efforts, and share information appropriately with the student and others campus partners working on the case.

A note on confidentiality

Bergen Community College highly discourages anonymous reports about students. Unless you include your contact information and submit this form, Bergen Community College may be limited in its ability to investigate the report or respond back to you regarding the subject matter. Once the report is received, a college staff member will review the report, and where necessary investigate the incident and determine appropriate intervention/action plan for the student’s behavior, which may include a meeting with the student and others involved in the incident. Please know that you, and the witnesses you identify in the report, may be contacted to provide further information.

With regard to reports of alleged Student Code of Conduct violations, student conduct records are protected by a federal privacy law called the Family Educational Rights and Privacy Act, therefore any information regarding the outcome any reports will not be shared without the direct consent of the student(s) involved.
